Thank you for your interest in EQUI-KIDS! We are thrilled that you or your loved one would like to get involved with our program. Part of the process to become an EQUI-KIDS or EQUI-VETS participant involves completion of our registration packet, which includes a number of documents that require completion. Please select the button below that best pertains to the prospective participant. After downloading and completing the packet, you may submit it to our Program Director by email, fax to 757.721.7354, or mail/hand delivery to the farm at 2626 Heritage Park Drive, Virginia Beach, VA 23456. Once received, your packet will be reviewed and program opening availability will be determined.
